Family Health Teams – A Primary Health Care Initiative of the Ministry of Health and Long Term Care, Province of Ontario

What is a Family Health Team (FHT)?

According to the MOHLTC, “a Family Health Team is an approach to primary health care that brings together different health care providers to co-ordinate the highest possible quality of care for you - the patient. Designed to give doctors support from other complementary professionals, most Family Health Teams will consist of doctors, nurses, nurse practitioners and other health care professionals who work collaboratively, each utilizing their experience and skills so that you receive the very best care, when you need it, as close to home as possible. “

Why FHTs?

As acknowledged by MOHLTC in 2004, “many Ontarians are having difficulty in accessing needed primary health care and in finding a family physician.” The Family Health Team initiative was launched to expand this access through interdisciplinary teams, with a focus on chronic disease management, health promotion and disease prevention, and extended hours of practice. Janus Global’s FHT success and clients

To support Ontario’s new primary health care strategy, Janus Global has helped launch 17 family health teams serving rural, urban and native communities across 9 Local Health Integration Networks. We have conducted research, facilitated provider and stakeholder groups, and recruited and mobilized new health teams. This far-reaching initiative has included provincial approval for significant new financial and related FHT resources, the development of new models for business and operational planning, information technology, clinic operations, and community partnerships.
